What we collect on this website
When you chat with Ava, text us, or book online, we collect only what is needed to hold an appointment: your first name, your mobile number, whether your visit is for everyday pain or a car accident, the date of the accident if there was one, and one short line about where it hurts.
We do not ask for your date of birth, Social Security number, insurance ID numbers, home address, medications, or medical history through this website or by text. The office collects that in person, on paper or in the practice's clinical records system.
How we use it
To schedule your visit, confirm it, remind you about it, and follow up if you do not book. We do not sell your information. We do not share it with advertisers.
Text messages
If you give us your mobile number, we may text you about your appointment. Message and data rates may apply. Reply STOP at any time to opt out and HELP for help. See our SMS terms for details.
Where it is stored
Conversations and appointment records are stored encrypted with our hosting and database providers, who are under agreements limiting what they may do with the data. Access is limited to Dr. Frank, office staff, and the vendor that operates this system.
Analytics and cookies
We record which page or phone number a visit came from so we know what is working. We do not use that information to build an advertising profile about you.
Your choices
You can ask us what we have about you, ask us to correct it, or ask us to delete it, except where we are required to keep clinical or billing records. Call the office at (689) 318-4081 or write to us at 5220 W State Rd 46, Sanford, FL 32771.
Notice of privacy practices
Once you become a patient, the practice's HIPAA Notice of Privacy Practices governs your protected health information. You receive a copy at your first visit and can ask for one at the office anytime.
